IAHHC Membership Application

This option is for new or renewing members who have operated a Home Health/Hospice for at least one year. You will choose which revenue level you are for your dues amount. A home health or hospice (may also offer personal services). These agencies will be operating in Indiana and will (1) supply in-home services to assist individuals with their activities of daily living and/or their health care or hospice needs; or, (2) that provide hospice inpatient services.

You will choose which revenue level you are for your dues amount. A personal service agency that operates in Indiana and will supply in-home services to assist individuals with their activities of daily living and/or their health care or hospice needs.

This option is for new members who are starting a new Personal Services Agency (This excludes agencies who have been acquired or combined). A personal service agency that operates in Indiana and will supply in-home services to assist individuals with their activities of daily living and/or their health care or hospice needs.

Please note that additional locations are locations that share your agency’s provider number and you agree to include their revenue when determining your dues. If an additional location has its own provider number, it does not qualify as be an additional location and must join as a Voting member.

This option is for new members who are starting a new Home Health or Hospice Agency (This excludes agencies who have been acquired or combined). A home health or hospice (may also offer personal services). These agencies will be operating in Indiana and will (1) supply in-home services to assist individuals with their activities of daily living and/or their health care or hospice needs; or, (2) that provide hospice inpatient services.

An entity that does not supply in-home personnel and does not supply goods or services for compensation to home care agencies, hospices, or their patients, but is interested in home care.
